Advance Directive
Living Will and Power of Attorney for Health Care forms are available to patients and family members. Call 217-876-4310.

Cancer Registry
The DMH Cancer Registry is designed to collect, manage and analyze cancer data. Each year, the registry sends letters to physicians and patients as a reminder of the importance of annual check ups and serves as a valuable tool for quality improvements and outcome measures. CarePages are personal web pages that allow you to send health updates and photos to everyone on your list at once reducing phone calls and stress.

Clinical Nutrition
Helping patients maintain good nutrition is important, especially during treatments. The Clinical Nutritional Department provides trained professionals who can counsel patients regarding their nutritional needs and offers cancer patients a variety of nutrition services provided by a Registered Dietitian. Clinical Research
To find the best treatment, researches will set up a special study called a clinical trial. Knowledge gained can lead to improved treatments, help develop new drugs and vaccines, help us learn how to prevent disease and improve quality of life for future generations. DMH Family Lodge
The DMH Family Lodge provides convenient guest housing for immediate family members of DMH inpatients. The lodge is located just southeast of DMH at 2254 N. Union St. To check room availability, talk to your hospital case worker or call 217-876-2020.

DMH Medical Equipment
Dedicated to providing quality equipment and supplies, our trained professional team assesses your individual needs and can instruct you on the use and maintenance of home medical equipment and supplies. A plan of care is then developed by you, your physician, and the home care team. For more information, call 217-876-4040.

Hospice Services
The primary goals of Hospice is to maintain quality of life and provide pain and symptom management, care and support for terminally ill patients and their families; 24-hour on-call service. Call 217-876-6770.

Mammography Initiative The DMH Mammography Initiative provides funding for no-cost mammograms based on income guidelines. For more information, call 217-876-1111.

Patient Navigator
A Patient Navigator works with many health agencies to ease the burden on cancer patients and connect them to community resources. For more information, call 217-876-6600.

Pastoral Services
For many, dealing with cancer is a time of great spiritual need. The DMH Pastoral Services provides spiritual support and counseling for patients and family members. For assistance, call 217-876-4320.

PrimeTime Patient Transportation
DMH PrimeTime provides members and senior citizens with free transportation to and from non-emergency DMH medical service facilities in the Macon County area. Van is available from 8 a.m. to 4:30 p.m. Monday-Friday by appointment only. For more information, call 217-876-2191.

Prosthesis/Bra Bank
A free limited, gently used prosthesis and bra bank available at the Women's Health and Breast Center. For more information, call 217-876-4377.

Smoking Cessation Classes
Learn to quit smoking with smoking cessation classes offered through Shore (Safety Health and Occupational Rehabilitation for Employees). For more information, call 217-423-3100.

Social Work Services
All DMH patients have access to a Social Worker who can help with a variety of practical, emotional, financial and social needs. For more information, call 217-876-4735.

Women with Disabilities
An extension of the DMH Mammography Initiative, Women with Disabilities provides no-cost mammograms to women with disabilities who qualify. For details, call 217-876-1111.

Wig Bank
A Wig Bank is available at Cancer Care Center of Decatur. For information, call 217-876-6600.

Women's Health and Breast Center
Uniquely designed and specifically tailored to help a patient through her plan of care, the DMH Women's Health and Breast Center furnishes resource and educational information and helps facilitate communication among healthcare professionals, the patient and family.
