Geriatric Assessment Center
What is the Geriatric Assessment Center?
The DMH Prime Time Geriatric Assessment Center is an innovative outpatient assessment program designed to assist individuals 65 and older (and their families) by identifying health needs and providing assessment, planning and intervention.
An assessment includes a thorough interview with the patient and his or her family (by a Gerontological Certified Registered Nurse), a physical examination performed by a Geriatrician (a board certified physician with additional training and expertise in Geriatrics). Additional evaluations and assessments may be completed by other healthcare professionals, including a neuropsychologist, a physiatrist, registered dietitians, pharmacists and case managers. After the assessment is complete the interdisciplinary team members meet with the patient and family to discuss a treatment plan.
What problems should be evaluated?

any of the following symptoms/concerns:
- Memory problems/dementia
- Falls
- Social isolation
- Depression
- Changes in dietary habits
- Difficulty with activities of daily living
- Difficulty managing alone at the home
- Families considering placement in an assisted living facility or nursing home
Who can refer?
Primary care physicians, adult children, and family members who have concerns about an elderly individual may request an assessment. Of course, self-referrals are also welcome.
What are the goals?
- To provide access to care for the geriatric population of our area through an interdisciplinary treatment plan outlining recommendations for the individual, family and primary care physician
- To address the concerns of individuals, families and primary care physicians
- To provide much needed assistance in referrals to appropriate community resources
- To help individuals achieve improved functional status and help assure more appropriate medication use
- To help ensure more appropriate placement in extended care facilities, etc.
